Prepare For Your Visit
We want to help you get the answers you seek. In order to do this, we have some recommendations to help you be prepared for your visit with us.
1. Before Your Visit
You will receive an email from us with some questionnaires to be completed. Please be sure to complete these prior to those visit with as much accurate information as possible. This information will be very useful to us in helping to provide you with a diagnosis and treatment recommendations. If you have any problems completing these forms please give our office a call at 212-606-1004 and we will be happy to help.
2. What To Bring To Your Visit:
A) Please bring all relevant medical records related to your condition for which you are being seen. Important examples are reports from previous injections, operative reports and reports for any radiologic studies you have had.
B) Remember to bring any CD’s from any images that you have had. If you have had any x-rays, MRI, CT or CT/Myelogram studies Dr. Albert will want to personally review the images (film or CD is fine) in addition to the reports for these studies.
C) A list of current medications and supplements that you are taking.
D) Insurance cards and a valid photo
3. Important Questions We Will Ask:
A) How long have you been having these symptoms?
B) What are your current symptoms? Does the pain radiate?
C) Are you experiencing any weakness in your arms or legs?
D) What makes your symptoms better or worse?
E) What treatments have you had in the past? What worked and what did not work?
F) Have you had previous surgery to the area? If yes, when? What was done? Did the symptoms improve?

If your particular insurance plan is not listed, you may still have coverage subject to the availability of ‘out-of-network’ benefits. Please do not hesitate to contact our office if you have questions regarding your insurance coverage.
